Insurance information and bill pay
Payment for services must be rendered at the time of your visit. This includes co-pays, deductibles, and coinsurance. As a convenience for you, we accept major credit cards (Visa, MasterCard, and Discover), cash and checks.
Based on your health insurance plan, we will attempt to determine the appropriate lab or facility for studies performed outside our office. However, it is ultimately your responsibility to notify us of any lab or facility that has preferred status with your health plan.
